When you can't do this anymore
It is not a failure. It is a limit.
Sometimes the honest answer is: I cannot keep doing this. Not because I don't love them, but because one person can rarely carry a whole care system. There are options, and you are allowed to use them.
Step away from the alone-ness first
Before you decide anything, please consider ringing Carer Gateway. Not because they have a magic answer, but because you don't have to make these decisions alone. They have walked many other families through exactly this.

Try respite first, even if it feels too late
Some carers find that a week of residential respite gives them enough breath to keep going for another six months. It can be a circuit-breaker, not a permanent decision.
Looking at residential aged care
If the person you care for has been approved for residential care — through the Single Assessment System, formerly ACAT — the move can be planned (or it can happen suddenly after a hospital admission).

Many carers describe relief and grief side by side after a move into care. Both can be real. Neither cancels the other.
You have not failed
Choosing more help is not the same as choosing to stop loving them. You are still their family. You are still their person. You can still visit, still hold their hand, still be there. You are just allowed to not be the only one carrying it anymore.
